What is
a Matching Gift Program?
The MassMutual Matching Gift Program is designed to recognize and demonstrate appreciation for the work carried out by non-profit organizations throughout the United States. The program serves to encourage and match associates’ charitable contributions to educational, health care, environmental, arts, cultural, community, civic, and social service organizations. As a national company, MassMutual is committed to providing support that enables non-profit organizations to better serve their local communities. We encourage our associates’ participation in this effort through the Matching Gift Program.

How do I participate in The
MassMutual Matching Gift Program?
As an eligible participant, you would make a personal donation of at least $25 to a qualifying organization. Gifts may be paid by check, credit card or negotiable securities that have a publicly listed market value. Insurance premiums may also qualify.

How
do I register
a gift?
Gifts may be registered via the Internet
www.givingprograms.com/massmutual, or by Form.
For both the web and telephone registrations, the Matching Gift Center will validate your eligibility and verify your gift with the organization. Once approved, the matching gift will be sent directly to the organization during the next scheduled distribution.

Complete Part I of the form and send the entire form with your contribution to the organization.

Upon verification of the eligibility of the donor, gift and organization, The Matching Gift Center will issue a matching gift and will notify the donor that payment has been made. |

Forms must be received by January 31 for previous year gifts to be accepted for processing. |

When is my gift matched?
Your gift will be matched when the Matching Gift Processing Center receives verification from the non-profit organization. The Matching Gift Center must receive verification within 180 days of the date the gift is registered. These matching payments are made quarterly, in March, June, September and December. For example, if you register a gift in January and it is verified by the non-profit organization in February, it will be matched in March. If your gift is not verified until April, it will be matched in June.

What is the annual limit?
MassMutual’s matching contribution will not exceed $5,000 for Agent of the Year, Chairman, President General Agents and Executive Vice Presidents; $1,500 for Other Executive Officers; and $1,000 for all other eligible Home Office and Field Associates, per calendar year. Annual limit for eligible part time Home Office Associates is $750. Leaders Club qualifying agents who are members of the Blue Chip Council are entitled to a $2,000 limit and Top of the Council to a $3,500 limit. These levels must be reached each year in order to qualify for the increased matching gift limit.

What is a 501 (c) (3)?
A 501(c)(3) is a section of the Federal Tax Code that establishes the criteria for tax-exempt charitable organizations. An organization must have this status in order to receive matching gift funds. This status allows donations and funds received to be tax-deductible.

If my name changes, how do I update my profile?
You must contact your Human Resources department to make this type of change to your profile.
